Tony - thanks for looking.

No routing at all, just what you see, no loopbacks etc

R5--------------------------R4
192.168.1.1 192.168.1.2

The counter doesn't get reset, it does count down from 120 seconds and then
clears down the line normally (n.b. paste taken from R4).

01:41:32: %ISDN-6-DISCONNECT: Interface BRI0/0:1 disconnected from 325500
R5, c
all lasted 121 seconds

I have no problems when I go back to legacy dial string/map.

I have added your lines, everything still the same.

Interestingly when I add the dialer idle-timeout directly onto the dialer
interface it works, however I can't see why the map-class isn't working.
