This is telling you that "one" of the chips (chip 2) in bank 1 is bad.

  Chip Bank Code Size Name
    1 1 01AD 2048KB AMD 29F016

    4 1 01AD 2048KB AMD 29F016

See where all the other chips say AMD and the other one does not.

Don't worry about the rom issue that I told you about earlier.

Your "show version" output told me you are already running 11.x roms.

Two thing you can try - first try to reset the flash memory simm.

Maybe this will work but I doubt it. You could try to clean the

simms contacts (gently) to see if that will help.

Second - and the more likely solution is that you have a bad flash simm.
