Go to http://www.arin.net/regserv/asnguide.htm - that's the main page for
ASN registrations through ARIN.

Basically what you need to do is fill out the ASN request template
(http://www.arin.net/regserv/templates/asntemplate.txt) and mail it to
ARIN. The form is more or less straightforward. They'll need to know why
you want an ASN -- mutlihoming to multiple ISPs is still an adequate
reason. You'll also need to provide contact information for yourself and
for each of your upstream providers.

After they get the form, ARIN will send you a services agreement that
you'll need to sign and return. When all that's done and you've paid them
$500, you'll get your ASN. It's $30/year to maintain it after that.
