The Call Manager product from Cisco is supposed to be at or moving to open
API's. There is a product that used to be called Com2000 is NT and open. Not
sure what it is being called today.

Routing between traditional PBX's and VoIP can be done by connecting trunk
ports of the PBX into VoIP enable routers.
