Your boss is probably responsible for many people. So when you go and ask
for a raise, he/she will definitely NOT remember all of your
accomplishments. Therefore, make sure you document every project that
you've worked on and any and all accolades you received by completing those
projects. Type this up in an organized Word document, recommended 1 page,
and ask your boss for a time when you can sit down with him/her. During the
meeting, present your reasons why you feel you deserve a raise and hand over
your achievements letter. Discuss the projects you've worked on and
emphasize on any things you've done "above and beyond" the average employee
(ie additional certs, awards, etc).
After you present your case your boss should happily give you a raise. If
he/she doesn't, in this industry, leave the company and find an employer
that will respect and award you for your accomplishments.

Additional note: DO NOT mention the "life changes e.g. (new born)." This
is a personal issue and while it may mean a lot to you, it in no way will
reflect on WHY you should receive a raise. Don't even mention it. Raises
are given for exceptional performance based on position. They are not based
on personal issues such as new house, new kid, being broke, etc. Focus on
your accomplishments and you'll do just fine.
