You can use the remark command. This command was first supported in IOS
12.0(?)T I believe.
router#config t
Enter configuration commands, one per line. End with CNTL/Z.
router(config)#access-list 10 ?
deny Specify packets to reject
permit Specify packets to forward
remark Access list entry comment
If you are running an older IOS version about all that you can do is use
named access-lists and make your remark part of the name.
